Binge Eating Disorder (BED)
Binge Eating Disorder (BED) is a severe, life-disrupting eating disorder characterized by recurrent episodes of eating large quantities of food (often very quickly and to the point of discomfort); a feeling of a loss of control during the binge; experiencing shame, distress or guilt afterwards; and not regularly using unhealthy compensatory measures (e.g., purging) to counter the binge eating. It is the most common eating disorder in the world.
Meaning
Binge Eating Disorder involves frequent overeating during which a person feels a lack of control and inability to stop eating. Unlike bulimia nervosa, those with BED do not regularly use purging behaviors, such as vomiting, excessive exercise, or fasting, after bingeing.
Symptoms
- Frequent Episodes of Binge Eating: Consuming an unusually large amount of food in a short period, usually done in secrecy and accompanied by a feeling of loss of control.
- Emotional Distress: Episodes are often triggered by stress, sadness, boredom, or anxiety and are followed by feelings of guilt, disgust, or depression.
- Absence of Regular Compensatory Behaviors: Unlike bulimia, there is no regular use of behaviors to prevent weight gain after binge eating.
- Physical Complications: BED can lead to obesity, cardiovascular disease, type 2 diabetes, and other metabolic conditions due to the pattern of eating.
Forms
BED is typically classified by its frequency and duration:
- Mild: 1–3 binge eating episodes per week.
- Moderate: 4–7 episodes per week.
- Severe: 8–13 episodes per week.
- Extreme: 14 or more episodes per week.
Treatment
Effective treatment for Binge Eating Disorder typically involves a combination of psychological, behavioral, and sometimes medical interventions. Here are the common approaches:
- Psychotherapy
- Cognitive Behavioral Therapy (CBT): This is the leading treatment for BED and involves helping individuals understand the thoughts and feelings that influence their behaviors, then developing healthier eating patterns and coping strategies.
- Interpersonal Psychotherapy (IPT): This therapy addresses interpersonal issues and improves communication skills, which can help reduce binge eating behaviors.
- Dialectical Behavior Therapy (DBT): Originally designed for borderline personality disorder, DBT has been adapted for BED to help manage emotions and reduce stress-related bingeing by teaching mindfulness, distress tolerance, and emotional regulation.
- Nutritional Counseling: Registered dietitians help individuals develop a balanced approach to food, learn healthy meal planning, and establish regular eating patterns without restrictions that can trigger binge eating.
- Medication: Medications such as antidepressants or anti-anxiety drugs might be prescribed to manage symptoms of co-occurring mood disorders. Additionally, the FDA has approved the use of lisdexamfetamine (Vyvanse) specifically for the treatment of moderate to severe BED in adults.
- Support Groups: Joining a support group can provide a sense of community and mutual understanding.
- Weight Management Programs: Given the frequent but not inevitable association with overweight and obesity, structured weight management programs can be beneficial, focusing not just on weight loss, but also on addressing the psychological aspects of eating behaviors.
The key to treating BED is to address both the physical and emotional aspects of the disorder, providing a comprehensive approach that can lead to a healthier lifestyle and improved overall well-being. As with other eating disorders, the treatment should be tailored to the individual’s unique circumstances.
