Confabulation
Confabulation is a phenomenon characterized by the production of false or distorted memories or information without the conscious intention to deceive. Individuals who confabulate may provide fabricated or inaccurate accounts of past events, experiences, or personal details, often in a sincere and confident manner. Confabulation can occur spontaneously or in response to memory deficits, cognitive impairment, or certain neurological conditions.
Confabulation as a Sign of Mental Illness

Korsakoff’s Syndrome
- Confabulation is a hallmark symptom of Korsakoff’s syndrome, a neurological disorder resulting from severe thiamine deficiency, often secondary to chronic alcohol misuse. Individuals with Korsakoff’s syndrome may confabulate to fill gaps in memory caused by anterograde amnesia and executive dysfunction.
- Confabulation may occur in dissociative disorders such as dissociative amnesia or dissociative identity disorder, where individuals may provide false or distorted accounts of past experiences or personal identity as a result of dissociative states or identity fragmentation.
Psychotic Disorders
- Confabulation can occur in psychotic disorders such as schizophrenia, particularly during episodes of acute psychosis where reality distortion and disorganized thinking are prominent features. Individuals experiencing psychotic symptoms may confabulate to explain or rationalize their experiences or delusional beliefs.
- Acute confusional states such as delirium can involve confabulation as a result of cognitive impairment and disorientation. Individuals with delirium may provide inaccurate or nonsensical responses to questions or exhibit memory deficits and confusion.
Bipolar Disorder with Psychotic Features
- Confabulation may occur during manic or psychotic episodes in bipolar disorder, where individuals may experience reality distortion, cognitive disorganization, and memory deficits. Confabulated narratives or experiences may reflect the individual’s altered perception of reality during these episodes.
