Elective Mutism
Elective mutism, now more commonly referred to as selective mutism, is a complex childhood anxiety disorder characterized by a child’s inability to speak in certain social situations, despite being able to speak comfortably in settings where they feel secure and relaxed, typically at home or in the presence of close family members. This condition is not due to a lack of knowledge of the language or speech impairments but rather a severe psychological response to anxiety.
Symptoms of Elective Mutism
- Consistent Failure to Speak: Children with elective mutism consistently fail to speak in specific social settings (like school or with strangers) where there is an expectation for speaking, even though they may speak normally in others where they feel safe.
- Social Isolation: Due to their inability to speak in certain settings, children may appear socially isolated or excessively shy. They might avoid eye contact, be withdrawn, or seem unfriendly or standoffish to unfamiliar people.

Forms of Elective Mutism

- Situational Mutism: Where the child speaks only in very specific situations or settings and remains mute in all others.
- Selective Mutism: More commonly recognized as a form where the child speaks freely in comfortable environments but consistently does not speak in particular social settings like school.
Treatment of Elective Mutism
Treatment typically focuses on gradually reducing the child’s anxiety associated with speaking, using a combination of therapeutic strategies:
- Behavioral Therapy: This might include specific techniques such as systematic desensitization and exposure therapy, where the child is gradually and gently exposed to the feared social situation in a controlled and supportive manner.
